COTA Australia’s ‘Self-management in Home Care’ project was a ground-breaking initiative funded by the Australian Government from 2017 to its conclusion in September 2019. The COTA Australia website hosts numerous resources  freely available to both providers and consumers to help build a greater understanding and practical knowledge about self-management in home care.

This webpage contains helpful resources and tools for providers and consumers of home care packages. These resources were developed as part of the Increasing Self-management in Home Care project. On the list you will find guidance, processes and tools to help integrate self-management into your organisation or into your life as a consumer of home care.

CONSUMER self-management resources:

The Consumer Guide to Self-management is your complete guide to self-management in home care from a consumer perspective. You will discover how to achieve more control and autonomy in the day-to-day functions of your home care package in partnership with your provider.

The Consumer Self-management Toolkit gives you handy and practical tools and resources that cover all aspects of self-management to help, guide and support you. These two documents are companions and should be used together for maximum benefit.

PROVIDER self-management resources:

The Provider Implementation Guide is your complete guide to self-management in home care from a provider perspective. It is designed to prepare you and your organisation to deliver a model of care that enables consumers to achieve more choice, control and autonomy in the day-to-day functions of their home care package.

The provider’s guide to Developing a Self-management Agreement gives you helpful information to establish an agreement with your consumers that spells out who does what in regards to the day-to-day activities of self-management.

Literature Review for the Self-management in Home Care project

This literature review provides an overview of consumer directed care (CDC) programs and focuses on service system design and implementation strategies shown to contribute to successful self-management and positive outcomes. The purpose of this review was to inform the development, implementation and evaluation of the Increasing Self-management in Home Care Project being conducted by COTA Australia 2017 – 2019.

Final Project Evaluation Report

COTA Australia’s Self Management in Home Care project concluded in June 2019. The project examined a model for innovative delivery of home care services identifying competencies needed for different levels of self-management and posed questions to evaluate requirements, benefits and challenges of this approach. This Final Project Report evaluates the project model against the identified project objectives.
