Tools and resources have been developed as part of the Self-management in Home Care project and will continue to be uploaded onto this page until September 2019.

This webpage contains a comprehensive list of helpful resources and tools for providers and consumers of home care packages. These resources were developed as part of the Increasing Self-management in Home Care project. On the list you will find guidance, processes and tools to help integrate self-management into your organisation or into your life as consumer of home care. The information available here will also help you find further resources in the area of your interest.’

Consumer Readiness Checklist

Using this simple checklist check your readiness to start self-managing your home care package more autonomously. This list helps you to identify the skills you will need to do it successfully.

Consumer Self-Assessment Tool

A part of successfully self-managing your package is developing a strong understanding of all your care and support needs. This tool will help you to do that.

Consumer Prioritising Needs Tool

You may require a number of care support services and products in your life, this tool will guide you on how to prioritise them to get the most of your home care package funding.

Consumer Care Plan Review Tool

After reading through the Consumer Self-assessment Tool and the Consumer Prioritising Needs Tool, you may see that your Care Plan doesn’t match your most current needs and requirements. Using this Care Plan Review Tool you can evolve your care plan to more accurately reflect your current situation.

Consumer Task Allocation Tool

As a self-managing consumer you will need to take on new tasks and responsibilities that are not required in an agency-managed service model. With your provider you can decide what tasks and responsibilities you would like to take on. You may wish to start with less responsibility and take on more as you develop your skills in self-managing your package.

Spend Check Pocket Guide

As a self-managing consumer you will have more autonomy in purchasing the services and products that you need for your care. With your provider you will need to develop a clear decision making process for making purchases. Sometimes it’s difficult to identify whether a certain service, support or product can be paid for using the home care package funding. This Spend Check guide provides you and your provider with a clear process.

Quality Principles – Care and Services Schedule

The Quality of Care Principles – Care and Services Schedule is the legislative guidance for home care package funding purchases. This is where you and your provider will find the legislated criteria on what you can and cannot spend your home care package funding on.
